Customer authorizes voxnumber or its agent to obtain from anyone information about customer’s credit history. Since the first payment with the application I know and agree with all terms and conditions. By signing below and or fill up the voxnumber’s application , you acknowledge you are of legal age, have provided true and complete information, have a copy of the agreement and, if signing or activating on behalf of a corporation or other entity, are fully authorized to sign on behalf of or legally bind such entity. In the event of unauthorized signature on behalf of a corporate customer, you agree to be personally liable for the amounts not paid when due. voxnumber makes no warranties, express or implied, regarding the Service or Equipment, and our liability is limited in accordance with our terms and conditions and as permitted by applicable law. AUTOMATIC PAYMENT FRON CREDIT CARD: Please charge my monthly voxnumber service fees to my debit/credit card. By signing, I authorized voxnumber to debit my credit card in accordance with the terms and conditions. Undersigned customer hereby authorizes voxnumber to charge the above listed credit card number for payment of voxnumber and/or any service provided including and not limited such Long Distance Services, voxfaxondemand, voxinfosales, etc. The undersigned understands that the same terms and conditions normally governing the use of the credit card apply. The undersigned customer authorizes voxnumber and its agents to perform credit card checks and/or verify other credit or financial information submitted to voxnumber, where permitted. The undersigned represents that he/she has the proper authority to request Services from voxnumber. The above signed (“Customer”) hereby authorizes voxnumber to debit his/her credit card, noted above, periodically and in advanced, as payment for services as stipulated in this agreement and on the reverse side under terms and conditions. This authorization will remain in effect until voxnumber receives written notification of cancellation. In such event any amounts due will be debited from the credit card as a final bill. In the event that the credit card will be declined, Customer hereby agrees to pay for rendered services promptly either by providing an alternate credit card or by any other method, including collection charges, reactivation fees or any other expenses incurred. Customer will notify immediately voxnumber, to the address below, via e-mail  or via fax, if the credit card is lost, stolen or the number or expiration date changed. Customer agrees hereby that he/she has read and understood both this Application agreement, terms and conditions at www.voxnumber.com and this statement, therefore relinquishing the rights to request a charge back from the credit card issuing company. This agreement is governed by the laws of the State of Florida, USA. This is a legal binding Contract.
